The main question it aims to answer is:\n\n1\\. Does CGM-guided insulin dosing education improve insulin dosing behaviors and glycemic outcomes in adults with diabetes who do not routinely use carbohydrate counting?\n\nParticipants will be randomly assigned to either the C-CARBS intervention or usual care. Participants will complete study visits, questionnaires, and meal and insulin logs, and CGM data will be collected to evaluate changes in insulin dosing behaviors and glycemic outcomes.",

"briefSummary": "This study is a single-center, prospective, randomized, open-label clinical trial evaluating three commercially available closed-incision negative pressure wound therapy (ciNPWT) devices: NPseal, PICO, and Prevena, in adult patients undergoing primary total knee arthroplasty (TKA) who meet predefined high-risk criteria for wound complications.\n\nA total of approximately 300 participants will be enrolled and randomized in a 1:1:1 fashion to receive one of the three study dressings applied over the closed surgical incision at the end of the procedure. All participants will otherwise receive standard perioperative and postoperative care. The primary follow-up period will extend through 90 days after surgery.\n\nThe primary objective is to compare the incidence of 90-day surgical site complications, including superficial infection, wound dehiscence, seroma, and hematoma, across the three dressing groups. Secondary objectives include evaluation of device usability, patient experience, and device-related issues (such as loss of seal, alarms, tubing or pump problems, fluid saturation, and premature dressing changes), as well as assessment of the interaction between skin closure method and device performance.\n\nResearch-specific activities are limited to eligibility confirmation, informed consent, randomization, administration of a brief postoperative patient survey at approximately postoperative day 7 and/or dressing removal, provider assessment of the dressing at removal, and collection of clinical outcomes from the medical record. No additional blood draws, imaging, or invasive procedures are required for participation.\n\nThis study is designed to address a gap in the literature by providing head-to-head comparative data on commonly used ciNPWT systems in a high-risk primary TKA population, with the goal of informing postoperative wound management strategies in clinical practice.",

"briefSummary": "Metabolic syndrome is a major risk factor for cardiovascular disease and type 2 diabetes, and its prevalence in young adults is increasing. Early identification of individuals at risk may improve prevention strategies. This observational, cross-sectional case-control study investigates the relationship between serum asprosin concentration, the expression of six selected circulating microRNAs, and the components and risk factors of metabolic syndrome in young women aged 18 to 35 years. Two equally sized groups will be compared: women meeting NCEP ATP III criteria for metabolic syndrome and healthy age-matched controls. Participants attend a single study visit during which fasting blood samples, anthropometric and body composition measurements, blood pressure, and validated questionnaires assessing physical activity, diet, sleep quality, perceived stress, and quality of life are collected. The study aims to determine whether asprosin and the selected microRNA profile have diagnostic value as biomarkers of metabolic syndrome in this population.",

"briefSummary": "This study will examine whether computer-assisted vocabulary instruction in Spanish and English helps young bilingual children with developmental language disorder learn and use vocabulary. It will include 120 Spanish-English bilingual children in kindergarten or first grade. Eighty children with developmental language disorder will be assigned by chance to bilingual computer-assisted vocabulary instruction, English-only computer-assisted vocabulary instruction, or usual English-language school services. Forty children with typical language development will receive bilingual instruction without random assignment, providing a developmental comparison.\n\nThe program teaches 36 vocabulary concepts through 24 small-group sessions, ordinarily over about six weeks. School-calendar disruptions may extend the schedule. Vocabulary will be assessed in both languages before intervention, immediately afterward, and approximately six weeks after the final session. The primary outcomes are naming and defining the taught concepts in each language.\n\nThe implementation component anticipates 15 unique school speech-language pathologists or appropriately supervised speech-language pathology assistants across five project years, with a planning target of approximately 3 new clinicians per year. Annual recruitment may vary with eligible caseloads. This component examines delivery fidelity, feasibility, clinician confidence, usability and satisfaction. Teacher and parent feedback about children's vocabulary use, communication confidence and engagement will also be collected after intervention and at follow-up where feasible.",

"briefSummary": "Researchers are looking for other ways to treat breast cancer (BC) that is hormone receptor-positive and human epidermal growth factor receptor 2-negative (HR+/HER2-) and either unresectable locally advanced or metastatic.\n\n* HR positive (HR+) means the cancer cells have proteins that attach to estrogen or progesterone (hormones) which help the cancer to grow and spread\n* HER2 negative (HER2-) means the cancer cells have a low amount of a protein called HER2\n* Unresectable locally advanced means the cancer cannot be completely removed by surgery and has spread into nearby tissue or muscles\n* Metastatic means the cancer has spread to other parts of the body\n\nTreatment for this type of breast cancer usually includes endocrine therapy (ET) and sometimes a second treatment. The main goal of this study is to learn if people who receive patritumab deruxtecan (also known as HER3-DXd and MK-1022) live longer overall or without the cancer growing/spreading, compared to people who receive chemotherapy or a different drug called trastuzumab deruxtecan.",

While the efficacy of MTM needs to be established, given the nature of the population served by MTM, it is important to design the study to enhance the external validity/generalizability of the findings as much as possible. The investigators will recruit 200 participants from the referral list of Community Servings, a Boston-based MTM provider, and randomly assign them to MTM (N=100) or usual care + monthly food subsidy (N=100). MTM intervention will consist of 6 months of weekly delivery of 10 ready-to-eat meals (approximately half an individual's weekly food intake), along with telephone-based lifestyle intervention that prepares participants for the post-treatment period. The usual care + food subsidy recipients will receive usual care along with 6 months of a $40/month food subsidy.",
